Clinical Study of the Acute Septic Arthritis of the Hip in Infancy and Childhood



Abstract
The acute septic arthritis of the hip is a primarily of the young and early diagnosis and immediate arthrotomy with debridement of the joint was the most essential part of management.
The nineteen patients of acute septic arthritis of the hip treated between 1975 and 1979 at orthopaedic department of the Korea University Hospital were followed for at least 6 2 months (averag 20 months). The clinical result were evaluated as excellent, good, fair or poor.
The results were as follows:
1. Of nineteen cases of the acute septic arthritis of the hip, the results were excellent in 15 cases, good in 2 case, fair in 1 case and poor in 1 case. (and 89% rate was above good result)
2. In the over-all series males predominated by a 1.5:1 rate, and children between one and three years old were 7 cases and more common than other age groups.
3. In the causative organisms staphylococcus aureus was the most prevalent with 800 of organisms cultured from synovial fluid.
4. Surgical drainage and antibiotic theraphy was done in all 19 cases and subsequent curettage in a case assocciated with osteomyelitis.
5. The excellent or good result were observed in the cases of only joint space widening or soft tissue swelling, without associated bone change Qn the X-ray examination, above one year old age, and early diagnosis and treatment within 5 days after onset of symptoms.
